Seminole County Public Schools Transition Continuum is dedicated to assisting students with disabilities achieve their goals for the future. The high school Individualized Education Plan (IEP) team and district staff work in conjunction with parents and families to provide links to services and community resources that can assist students with moving successfully through the continuum and beyond to life after high school.
Please note: The continuum below will vary depending on the needs and abilities of each student. Diploma track designations are included to differentiate between course and service offerings.
